The lady on the right with the pen is Rose, she is an anaesthetic officer in Kampala. These anaesthetic officers are not trained doctors but have had specific training in anaesthetics to a level good enough for most basic operations.

They are a very important part of the medical system in many countries.
Here she is writing notes as the operation goes on, and operation to repair a cleft lip on a child.
Cleft lips are so easy to repair and make a lifetimes difference to a child (or an adult). In this case a charity 'Smiletrain' is funding the operation.
