Flies I can tolerate, but ticks, surely he would see the ticks and pick them off.
A great human condition portrait though, a very direct stare he is giving you.
Good contrast, and the out of focus background does not detract for the face.

The image is quite beautiful in black and white. His bone structure is so delicate and his condition from the ticks is heartbreaking. It is a truthful and honest image.
