Forms required for pics to for publication - help requested

Hi TE'ers,

I'm doing some photowork for youth red cross in Denmark. We need a standard form of the sort that the motive signs whih allows that the pc is being used in public contexts.

I don't even know what such a document is called, but htere must be some standard forms out there. Do anyone knows where to download them. I'd be especially appreciated if anyone knows of a danish one, but any hints would be great!
